Roster, Liability Waiver Information and
Concussion Information
In accordance with League Rule
B-2, all coaches must submit a complete roster, as well as an original, hard
copy, signed "Consent for Medical Treatment and Voluntary Release,
Acknowledgement and Acceptance of Risks Indemnity Agreement" (hereafter
"Liability Waiver") for each player.
New in 2012-2013:
In April of 2012, Wisconsin Act 172, the "youth
athlete concussion law," was enacted. For the 2012-2013
season, parents/guardian, athletes and coaches must also sign an
agreement stating that they have read and understand information
concerning concussions. The Parent & Athlete Agreement and the
Coaches Agreement are available at the links below. The
appropriate agreement must be signed and on file with our office for
every player and coach participating in The Great Northwest Basketball
League.
All forms must be submitted
to the League via email or hard copy by USPS. All rosters,
Liability Waivers and Concussion Information Agreements are due in to our office at least one day before your first 2012-2013 meet.

team's schedule.
Every player on every team must
submit a newly-executed
Liability Waiver each season before teams play
their first game.
Coaches should keep a
photocopy
of the Liability Waivers in their coach's bag that they take to games in
case one of their players is seriously injured and a parent or legal
guardian is not present.
In a non-life threatening situation,
hospital and EMT staff may refuse treatment without a signed consent for
medical treatment provision, which is part of the Liability Waiver form.

ROSTER FORM
We can accept a team's roster on another sheet of
paper (for example, a computer print out of the team's information.
However it must include the following items:
Player name (first and last)
Player's jersey number
Player's home address
Name of School player attends (in the 2012-2013
school year)
Player's grade in the 2012-2013 school year

LIABILITY WAIVER FORM
Every player on
